Abstract

This IDC study analyzes the City of Leeds' approach to integrating health and social care data. Leeds is a front-runner in the integration of health and social care services. The City of Leeds provides lessons on how to align strategic vision, culture, process, technology, and data to move forward the agenda of health and social care integration.
